Cervical cerclage is a surgical procedure where stitches are used to close the cervix during pregnancy to prevent premature birth . It is done in women with cervical incompetence to prevent early changes in a woman’s cervix and therefore prevent premature labor. These sutures are typically removed around 37 weeks when the baby is considered full term although it may be removed earlier if necessary. If you need a cervical cerclage in one pregnancy, it is possible you will need it for future pregnancies.
Why is a cervical cerclage done ?
This procedure is done if a woman’s cervix has the tendency to open under the pressure of the growing pregnancy. Cervical incompetent may be the result of
- history of second trimester miscarriage
- damaged cervix due to termination of pregnancy
- a previous “cone biospy” or a “LEEP” procedure
Your doctor may recommend a cervical cerclage if you have:
- short cervix
- history of second trimester miscarriages or premature birth
- dilation of the cervix with a visible amniotic sac before week 24 of pregnancy (emergency or rescue cerclage)
When is a cervical cerclage done ?
The best time for this procedure is around the third month (12-14 weeks). However some women may need a cervical cerclage placed in later pregnancy, in which case it is known as an emergency cerclage. Emergency cerclage is done where opening or shortening of the cervix has already begun. In such cases, you will require a cervical cerclage in future pregnancies.
If changes in cervix are found very late in pregnancy or if the opening of the cervix is significant, bed rest may be the best alternative.
What are the benefit of cervical cerclage ?
In women with weak cervix, this procedure prevents miscarriages or premature birth. Cervical cerclage is very successful in about 85 to 90% of cases in treating cervical incompetence. However cervical incompetence can be difficult to diagnose.
